Treatment Consent and Liability

By booking a treatment with Seren Haus you confirm that you have read and agree to the following. Before any injectable or advanced treatment you will also have a consultation and sign a treatment-specific consent form.

Your information

You are aged 18 or over.
The medical and personal information you give us is complete and true, including medical conditions, medications, allergies, pregnancy or breastfeeding, and any previous treatments.
You will tell us about any changes to your health before each appointment.

Risks and results

Every treatment carries some risk. Common side effects such as redness, swelling, bruising and tenderness will be explained to you at your consultation, along with rarer risks.
Results vary from person to person and cannot be guaranteed.
Some treatments need a patch test before your appointment. If one is needed, the treatment cannot go ahead without it.
Your practitioner may decide on the day that a treatment is not suitable or safe for you. If so, it will not go ahead.

Aftercare

You agree to follow the aftercare advice you are given.
If you have any concerns after your treatment, contact us straight away on 07511 719042 or enquiries@serenhaus.co.uk. In an emergency, call 999 or go to A&E.

Liability

Seren Haus is not responsible for problems caused by information you did not tell us, or by not following aftercare advice. Nothing in these terms limits liability for death or personal injury caused by negligence, or any other liability that cannot be limited by law. Your statutory rights are not affected.
